Is this a good one, do you think? Should I add anything?

Maybe something to make sure they're aware of medical costs associated with owning a parrot?

Ah ok. I'll add that in. Thanks Bea! :)

Any other suggestions are welcome! :D

I added that in. I also added "Those under 18 need to have parental permission to adopt a parrot. Is this ok?" because I don't want kids adopting without the parents' knowledge, and then the parent just releases the bird or dumps it at the nearest pet store.

Looks good I would also put something about geting the proper size cage. Or will they get the cage when they adopt the parrot?

Looks good, for people that have never owned a bird maybe something like do they know about bird nutrition and there diets, some people still think as long as you give them seed there ok

Everyone's suggestions are great- Make it in a way where you are asking the question so then you can get an answer and get a feel for the person rather then a multiple question....If you need me to dress it up after you're done editing let me know-I'd be glad to do it.

OR write it as an HTML so it's an actual form that they can just submit.

I tried to, but the stupid Freewebs form only allows one thing. It is so stupid. I searched for others but they are WEIRD...lol:blink:

I'll try to get a form ASAP. :)

I will put in a question asking what the diet they would feed would be, and the cage size. ;)

I was just wondering if you ask people why they need to rehome their parrots, Iam sure you do. But I just thought that way you can let the adoptive parents know if they are going to get a screamer ect. So there won't be any surprises.

Yes, when someone e-mails me I ask why they are re-homing it. :)

This tiel is being rehomed because his owner was keeping him for a friend, but the friend doesn't want him anymore. They say he's a screamer and a biter, but he could just be using the regular amount of tiel noise.
